A Qualified social care worker with a wide range of experience within the Voluntary sector. Able to work on my own initiative as well as part of a Team. Committed to maintaining and promoting high quality standards. Proven track record of identifying individual Development Plans, as well as to ensure the standards are met and adhered to, that are laid down by the care commission and other outside agencies. Experienced in ensuring that a person centred approach is used at all times, to ensure that people have the support they want, and the life they want in the community.

Context
I work for a organisation, which has over 500 employees, with a wide variety of support services for people who have various support needs, i provide visiting support, supported living, own tenancies, care at home . Some of the people I have worked with have moved from long term institutions, into their home for the first time, people came with negative reputations and very little skills due to lack of opportunities. The situation currently being that people are now all well known and actively involved in their local community, and are valued citizens who have a say , ensuring full participation and giving clear, focused direction to people hence developing the very successful services along the way. I encouraged active participation and full involvement from people supported and family members and advocates at all times.
With my main focus for the service user is to ensure people have the life they want.
I did work a variety of shift patterns including sleep-over’s, night shifts and on-call.

Responsibilities
To support service users to protect and maintain their tenancy agreements. Support service users to use their rights related to the tenancy and to promote the service users acceptance as a good neighbour in their community. To build relationships with the person you are supporting, other agencies and their family, based on trust and respect. Provide agreed levels of care and support to assist people to maintain and develop their skills and abilities both practical and personal. This includes bathing and personal hygiene. Carry out all household tasks, including cooking, laundry and shopping, while maximising service user participation in these tasks.
Assist and/or take responsibility for the service user’s medication. This may include, for example, administration of suppositories and Rectal Diazepam. Keep records which enable a consistently high standard of support to be provided and where necessary, incident reports and other documents and inform my line manager of changes and new risks.
I am responsible for assisting the service user in the management of their day to day finances and the enabling of the service user’s management of their household budget and annual spending plans.
I assist the service user in planning and achieving positive goals in their life by achieving new integrated activities in their own community.
I support service users to seek opportunities for them to make connections with people in the community, and support the growth of real friendships over time.
I work in ways that enhance the individual’s status and role in the community and upholds their rights as a citizen and promote a positive view of the person and know how to respond to negative attitudes.


Major Achievements
I have facilitated people both moving in from the parental home, as well as moving on to other specific providers or secure accommodations. I’ve made referrals to other professionals, and provided the correct supports and networks to be in place for people.
I get satisfaction knowing that people have adapted and believed and put great efforts into their lives to develop and move on, knowing that I felt and evidenced that I have made a difference to peoples lives is what matters.
Been involved in a lot of transitional moves for people, and have been requested for to carry this out.
Been asked to go into area and sort out services and to get them running up to Care Commission Standards.
Completed my SVQ Level 2 in Social Care
Been transferred many times within my organisation to support other teams to provide better services.
I Support one specific female who had the negative label of having challenging behaviour. Supported her to break down these negative barriers and be looked at as a valued member of the community.
