SVQ 3 in health & social Care essential and a car owner/driver essential. Experience in Home Care essential. Ability to work shifts and weekends also essential. Local person preferred for this post.

Transitions Care Ltd are an independent service provider delivering high quality care at home and housing support services to vulnerable adults in Central Aberdeenshire. The above post is challenging but a dynamic and enthusuastic individual will thrive facing the demands of the post.

Responsibilities include Assessing, Planning, Implementing and Reviewing services and individual support plans. Meeting with other agencies involved in service user's care packages and their families or other representaives.
Duties also include interviewing and induction of new staff and supervision of established staff. Maintaining and building team morale, delegating responsibilities to junior members of staff to maintain high quality service provision and taking direction from the senior management.

Transitions Care Ltrd operate a 24hr response and an on call system is operated with the Care Services Supervisor responsible for evaluating this system.

Salary £18,000 for an average 39hr week
